I am aware that participating in a fitness program poses some risk and I know of no physical or other reason why I should not participate.  
​This will acknowledge that neither the Instructor nor Zumba Fitness LLC is responsible for any injury I might sustain.  
PHOTOS & VIDEOS
Photographs and videos taken at this event become the property of Love It Fitness.  I consent to the use of photos and/or video clips in which I appear.
COVID SCREENING
This will confirm that I have not tested positive for Covid-19 within the last 10 days or been exposed to anyone who has tested positive for the virus,
nor have I travelled outside the province recently.  Please stay home if you are feeling unwell on the day of the event.
